Note: All Consents are deemed valid for the entire duration of your child’s stay at Kids Spot Nursery.

Health Details and Emergency Medical Situation

I, the undersigned, lawful parent / guardian of the child hereby accept and agree that in case of an accident or injury occurring to or disease being contracted by my child, the nursery manager, teacher, nurse, or any person in charge of the nursery shall have full authority to take the necessary decision to ensure appropriate emergency medical treatment of my child by the nursery nurse or if necessary at a hospital / clinic, If I cannot be reached at the emergency numbers.

In the event of an emergency or accident, I authorize Kids Spot Nursery to take my child to the nearest hospital / clinic for emergency medical treatment. The nursery will make every attempt to contact me or my emergency contact person.

We shall also keep the nursery updated of any changes in the health information of our child. We declare that we have provided the nursery with full relevant information and record about our child to the best of our knowledge. We understand that any incorrect or misleading statements, or omissions could affect our child’s admission at Kids Spot Nursery even after our child commences nursery. The nursery shall not have a responsibility in the event of any harm caused by any misleading information.

We have no objection for the nursery nurse to seek further information directly from our child’s doctor during emergency or when necessary.

I hereby authorize Kids Spot Nursery, my child’s care provider, to use the following medication products on my child according to manufacturer or physician’s written instructions. I will not hold Kids Spot Nursery liable for any allergic reactions or other symptom when the medications are used in accordance with the below instructions:

We love taking photos and videos to celebrate your child’s work and achievements. These images may appear on nursery website, social media, display boards etc.

Yes, I hereby give permission to take my child’s photograph. No, I do not give permission to take my child’s photograph.

I understand that my child should be ready and wait at bus stop location 5 minutes before the agreed pick-up time.

The nursery will not be held liable in the event of injury or accident while my child is in the bus unless a case of negligence is accepted.

We hereby agree that neither the nursery, nor any of its supervisors, teachers, assistants, officials, voluntary helpers, or owners, shall be held responsible for my child in the following cases:

Prior to actual delivery of my child into the custody of the teachers or officials inside the nursery grounds, or after my child has been collected from the nursery grounds by me or a person authorized by me to do so, on a normal nursery day.

Whilst on nursery grounds outside the official opening times.

Unless the injury is caused by or has resulted from a neglectful act or omission of any employee, teacher, nursery supervisor, nursery assistant or helper authorized to act for or on behalf of nursery.

To keep the nursery, its supervisors, teachers, officials, voluntary helpers, and owners absolutely harmless, fully, and effectively indemnified against all actions, claims, liabilities, images, expenses, costs, chares, fees (including medical, judicial, attorney’s fees), whatsoever, which are suffered by the nursery as consequence of any accidental injury or contraction of any viruses/by the child.

To indemnify and keep the nursery compensated in respect of any loss or damage to the property belonging to or in custody of the nursery caused by my child.

We have thoroughly read Kids Spot Nursery’s Admission Application and Policies and Procedures (Link to Policies and Procedures). We hereby accept all consent listed herein. We also confirm that all information in the Admission Form is true and accurate and agree that in the event the information provided was incorrect or inaccurate, the nursery reserves the right to take necessary action, including cancellation of admission. We shall keep the nursery continuously updated on any changes in the information related to our child or to ourselves. We agree to fully abide by all the nursery’s policies in support of our child’s well-being, including fee payments.
